NEW & RENEWAL PATIENTS
WITH MEDICAL RECORDS: $75
WITHOUT MEDICAL RECORDS: $99
Records must be dated within the last 12 months and include a physician assessment and plan. CT scans, MRIs and X-rays, DO NOT qualify as records. If you don’t have any medical records, one of our doctors will establish them for you. Keep in mind all applicants must pay a government fee of $150 dollars. patients with a valid EBT card qualify for a government fee of $75 dollars.
